The MH-C801D and MH-C808M chargers come with TWO separate adapter pieces, the AC Cord (the one that plugs into the wall and the Power Adapter) and the Power Adapter (the one with the brick that plugs to the unit).
NOTE for the MH-C808M Charger:
This adapter is for the MH-C808M VERSION 1 Charger. The back of the charger should look like the image below.
